Case Management at Destination Hope
When participating in rehab at Destination Hope, our case management department will work closely with you to manage any and all issues that stand outside of your addiction or mental health diagnosis. Our Case Manager will assist you in communicating with all necessary parties to help resolve and/or manage issues related to:
- Financial Services – Links with DCF for Food Stamps Benefits, assists with Social Security Application as needed, potential programs for indigent individuals.
- Legal Services – Communicates with probation officers, parole officers, attorneys, judges, and all individuals related to current or pending legal issues. Sends documentation confirming admission to, presence in, plans for, and completion of treatment at Destination Hope. Sends any documentation as needed to resolve or postpone legal matters until after treatment. Attends court with clients as needed to confirm progress as requested by judges, attorneys, etc.
- Employment Services –Links with vocational rehabilitation program, Pathways2Work. Assists with FMLA process (getting you approved to be in treatment as a medical necessity), Short Term Disability (getting paid from your job while in treatment less than 12 weeks), Long Term Disability (getting paid from your job while in treatment 12 weeks plus). Assists with resume development, interview coaching, interview attire, transportation, job placement, and potentially schooling to further credentials and foster career development.
- Discharge Planning – Arranges appropriate discharge plans including supportive housing such as substance abuse primary sober living, dual diagnosis sober living, mental health supportive housing and ongoing outpatient treatment such as individual therapy and psychiatry
